Специфические осложнения монохориального многоплодия оказывают влияние как на антенатальное развитие плодов, так и на период ранней неонатальной адаптации, что сопровождается нарушениями метаболического статуса.Цель исследования. Изучить метаболические показатели (по рутинным показателям биохимического анализа крови и метаболическим индексам) у недоношенных детей из монохориальных двоен в периоде ранней неонатальной адаптации.Материалы и методы. Исследование выполнено на базе ФГБУ «НИИ ОММ» Минздрава России в период с 01.01.2018 по 31.12.2024г. В исследование включены 1 230 недоношенных монохориальных диамниотических (МХДА) близнецов гестационного возраста 22/0-36/6 нед.Результаты исследования. Обнаружены различия между группами МХДА близнецов с осложненным и неосложенным антенатальным периодом по показателям: общего белка, г/л (1-я гр.- 41,0 (34,6-46,0); 2-й гр. — 40,0 (36,0-46,0); 3-я гр. — 44,0 (40,0-48,0); р&lt;0,0001), альбумина, г/л (1-я гр. — 29,3(25,3-31,9); 2-я гр. — 29,0 (27,0-31,4); 3-я гр. — 30,1 (28,5-32,5); р&lt;0,0001), АСТ ЕД/л (1-я гр. — 58,0(43,3-82,0); 2-я гр. — 52,0 (40,0-75,0); 3-я гр. — 43,0 (34,0-56,0); р&lt;0,0001), гипогликемиит (1-я гр.- 162 (67,5%), 2-я гр. — 180 (65,2%), 3-я гр. — 396 (55,5%) р=0,002 и р=0,006 соответственно) а также ряда биохимических индексов.Заключение. Представленная оценка метаболического статуса в группе недоношенных МХДА близнецов с осложненным антенатальным периодом установила существенные изменения изучаемых показателей, свидетельствующие о развитии неонатального дисметаболического синдрома.</p></abstract><trans-abstract xml:lang="en"><p>Summary. Specific complications of monochorionic multiple pregnancy affect both the antenatal development of the fetus and the period of early neonatal adaptation, which is accompanied by metabolic status disorders.The purpose of the study. To study metabolic parameters (according to routine indicators of biochemical blood analysis and metabolic indices) in premature infants from monochorionic twins in the period of early neonatal adaptation.Materials and methods. The study was carried out at the Research Institute of Obstetrics and Maternity Care of the Ministry of Health of the Russian Federation from 01.01.2018 to 31.12.2024. The study included 1230 premature monochorionic diamniotic (MCDA) twins of gestational age 22/0-36/6 weeks.Results and discussion. Differences were found between the groups of MHD twins with complicated and uncomplicated antenatal periods in the following parameters: total protein, g/l (group 1 — 41.0 (34.6-46.0); group 2 — 40.0 (36.0-46.0); group 3 — 44.0 (40.0-48.0); p&lt;0.0001), albumin, g/l (group 1 — 29.3 (25.3-31.9); group 2 — 29.0 (27.0-31.4); group 3 — 30.1 (28.5-32.5); p&lt;0.0001), AST U/l (group 1 — 58.0 (43.3-82.0); group 2 — 52.0 (40.0-75.0); 3rd group — 43.0 (34.0-56.0); p&lt;0.0001), hypoglycemia (1st group — 162 (67.5%), 2nd group — 180 (65.2%), 3rd group — 396 (55.5%) p=0.002 and p=0.006, respectively), as well as a number of biochemical indices.Conclusions.
